P003 MicroRNAs as biomarkers in Inflammatory Bowel Disease unclassified (IBDU) patient samples may predict the development from IBDU to Crohn’s Disease or Ulcerative Colitis

Abstract Background Differential diagnosis in Inflammatory Bowel Disease (IBD) is challenging; typically, discriminating Crohn’s (CD) from Ulcerative Colitis (UC) among IBD unclassified (IBDU) patients a diagnostic encounter cases where the inflammatory lesions are restricted to colon. MicroRNAs found be involved pathogenesis and also evaluated as biomarkers therapeutic targets IBD. Methods We obtained formalin fixed paraffin embedded (FFPE) colonic mucosal biopsies of 35 IBDU (including adults children), 8 (4 CD 4 UC) who were classified into or UC collected endoscopy. Based on patient charts, The samples reclassified CDs remained after minimum 3 years clinical follow up, ie. 12 (IBDU-CD) 15 (IBDU-UC). Eight IBDU. MicroRNA profiling was performed all using Affymatrix GeneChip miRNA Arrays. Arrays processed pd.mirna.4.0 oligo packages R analysed LIMMA package. Results that miR-3176 miR-182-5p upregulated IBDU-CD miR-671-5p, miR-668-5p, miR-4793-3p, mir-6776 IBDU-UC with p values less than 0.05 1< log [fold change] <-1 for 6 microRNAs. Higher expression levels compared samples, resulted an area under curve (AUC) 0.87 (95% confidence interval (CI), 0.71 – 1.00) 0.81 (CI, 0.63 1.00), respectively, IBDU-UC. On other hand, had AUCs 0.69 0.25-1.00) 0.21-1.00), UC. miR-671-5p higher AUC 0.48 - UCs, whereas IBDU-UC, only 0.64 0.42 0.87). Conclusion conclude differential microRNA can predict development UC, profile differ at time diagnosis. Our findings suggest IBDUs may have different pathogenic mechanisms IBDU-CDs IBDU-UCs.
